Referrals
Physician referrals are the responsibility of the patient; please obtain the required documentation before your appointment.

Forms of Payment
We accept payment in the form of cash, check, debit or credit card. We do not accept American Express.

Co-payments and Deductibles
Payment is required for all services at the time they are rendered. For managed care patients, applicable co-payments and deductibles will be collected. You are required to pay any unmet deductible, non-covered services and co-payments.

Nonpayment and Returned Checks
Nonpayment of your account balance will result in collections and credit damage. A $30.00 fee will be added to your account for returned checks.

Cosmetic Consults
Patients that have a cosmetic consult will receive credit in the amount of the consult toward their cosmetic procedure, if scheduled and performed within 6 months.

Cancellation Policy
Please provide at least 48 hours notice if canceling a surgery to allow another patient opportunity to fill your surgery date. Allow 24 hours cancellation notice if you need to reschedule a general dermatology appointment, follow up or full skin exam.

Botox, Restylane and Laser patients will be required to leave a credit card number at the time of scheduling. If cancellation is made 7 days prior to the scheduled appointment, there will be no charge cancellation. If the cancellation is less than 7 days prior to the scheduled appointment, the patients credit card will be charge $50 for each 15 minute appointment that they are scheduled for.

Schedule Changes
Please note that we try to accommodate everyone as quickly as possible. Patients with Melanoma will take precedence over other types of appointments. DSSC may call to move, reschedule or cancel your appointment based on the unpredictable nature of our business. Please understand that we are trying to meet the needs of our patients based on their severity.

Financial Options
Financing options are available to those who qualify. APR is dependent on credit history and ability to repay. DSSC uses a third party to provide financing options to its patients. In no way are we affiliated, owned or operated by the financing company or companies. DSSC assumes no liability to the approval or denial of financing or to the payment or lack of payment to the financing company.
